How The Peak Mark Clinic collects, uses, shares and protects personal information submitted through this website, and the rights you have over it.
Last updated
This policy explains what personal information The Peak Mark Clinic (“the clinic”, “we”, “us”) collects through this website, why we collect it, who we share it with, and the choices you have. It applies to this website only.
01Who we are
The Peak Mark Clinic is a private healthcare provider at 20 Sholanke Street, Akoka-Yaba, Lagos, Nigeria. For the personal information described in this policy, the clinic decides how and why it is used, which makes it the data controller.
You can reach the clinic through the contact form on the Contact page, or by visiting 20 Sholanke Street, Akoka-Yaba, Lagos.
02Information we collect
We only collect information you choose to give us, plus limited technical information needed to run the site.
- Appointment requests: your full name, phone number, email address, preferred date and time, the service or reason for your visit, and any additional message you write.
- Contact messages: your full name, email address, message, and phone number if you choose to add one.
- Technical information: our hosting infrastructure may log your IP address, browser type, device type and the pages requested, for security and reliability.
03Please keep health details brief
Information about your health is sensitive personal data. The “additional message” and “message” boxes are for short practical notes, such as the best time to call. Please do not include detailed medical history, test results or other sensitive details in these forms. Share those with the clinical team in person or through the clinic's secure channels instead.
04How we use your information
We use the information above to:
- respond to your messages and enquiries;
- review, arrange and confirm the appointment you requested;
- keep this website secure, working and free of misuse;
- meet our legal and regulatory obligations.
05Our lawful basis
We rely on your consent when you submit a form, on taking steps at your request before providing care, on our legitimate interest in running a secure, useful website, and on legal obligations where they apply. Where we rely on consent you can withdraw it at any time, as described under “Your rights”.
07Patient portal
When you choose to sign in or register, you are directed to the Purple Pluspatient portal, which is operated separately from this website. This policy does not cover information you enter there. Please read the privacy notices shown in the portal.
09How long we keep information
We keep enquiry and appointment-request information only for as long as it is needed for the purpose it was given, and for any longer period required by law or professional record-keeping rules. When it is no longer needed it is deleted or anonymised.
10How we protect information
We use reasonable technical and organisational measures to protect personal information against loss, misuse and unauthorised access. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.
11Your rights
Under the Nigeria Data Protection Act 2023 you have the right to:
- ask whether we hold personal information about you and receive a copy;
- have inaccurate or incomplete information corrected;
- ask us to delete your information, or restrict how we use it, where the law allows;
- object to our processing, including for direct marketing;
- withdraw your consent at any time, without affecting processing already carried out;
- receive your information in a commonly used format where the law gives you that right;
- complain to the Nigeria Data Protection Commission if you believe your information has been mishandled.
12Children
